Job Description
Care Manager coordinating person-centered physical, behavioral, and social health services for Trillium members. Managing assessments, care plans, providers, and community resources across South Central North Carolina.
Responsibilities:
- Serve as the primary point of contact for assigned members, providing ongoing care management and advocacy
- Conduct comprehensive assessments of member needs using person-centered planning methods and organizational tools
- Develop, implement, and monitor individualized care plans with members, families, and multidisciplinary support teams
- Coordinate services across providers, agencies, and community resources to address physical, behavioral, and social health needs
- Ensure assessments and care plans reflect changes in member needs and circumstances
- Work with primary care providers, specialists, care manager extenders, transition staff, and caretakers/legal guardians
- Remain available remotely from 8:30 a.m. to 5:00 p.m., travel within the community to meet members, and attend meetings as needed
Requirements:
- Must meet Qualified Professional (QP) status
- Fully licensed by the NC governing board regulating Human Services professions; OR fully licensed RN licensed to practice in North Carolina by the NC Board of Nursing with four years of full-time MH/DD/SA population experience; OR master's degree in a human service field with one year of full-time MH/DD/SA experience; OR bachelor's degree in a human service field with two years of full-time MH/DD/SA experience; OR bachelor's degree in another field with four years of full-time MH/DD/SA experience
- Substance abuse professional alternatives require supervised alcoholism and drug abuse counseling experience as specified
- If serving LTSS members, at least two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience
- Must have a valid driver's license
- Must reside within 75 miles of Trillium's South Central Region: Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, or Wayne counties
- Must be able to travel within catchment as required
- All candidates must pass a drug test as a condition of employment
