Job Description

Care Manager coordinating whole-person behavioral health services for Trillium Health Resources members. Managing assessments, integrated care plans, provider collaboration, and community-based member support.

Responsibilities:

  • Provide oversight, outreach, engagement, and support for members receiving Tailored Care Management
  • Facilitate comprehensive assessments of members' needs and evaluate results
  • Initiate and maintain integrated care plans addressing physical health, mental health, social support, lifestyle, community involvement, and risks to well-being
  • Serve as the single point of contact for members
  • Work with multidisciplinary care teams, including providers, specialists, care manager extenders, transition staff, and caretakers/legal guardians
  • Facilitate regular face-to-face and telephonic meetings with members
  • Consult with providers to gather and share information relevant to members' care
  • Work remotely between 8:30 am and 5:00 pm and travel within the community to meet members and attend meetings as needed

Requirements:

  • Must meet Qualified Professional (QP) status
  • Fully licensed by the NC governing board regulating Human Services professions; or fully licensed RN licensed to practice in North Carolina by the NC Board of Nursing
  • RN pathway requires four years of full-time experience with the MH/DD/SA population served
  • Master's degree in a human service field plus one year of full-time MH/DD/SA experience, or one year of supervised alcoholism and drug abuse counseling experience
  • Bachelor's degree in a human service field plus two years of full-time MH/DD/SA experience, or two years of supervised alcoholism and drug abuse counseling experience
  • Bachelor's degree in another field plus four years of full-time MH/DD/SA experience, or four years of supervised alcoholism and drug abuse counseling experience
  • If serving LTSS members, at least two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience
  • Must have a valid driver's license
  • Must reside in Trillium's North Central Region: Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrrell, Washington, or Wilson counties
  • Must be able to travel within the catchment as required
  • Must pass a drug test as a condition of employment
