Job Description

Care manager coordinating transitions, care plans, and community services for Trillium Health Resources. Supporting North Carolina individuals with behavioral health, substance use, TBI, and IDD needs.

Responsibilities:

  • Manage ongoing care for assigned individuals to support continuity and stability across services and settings
  • Coordinate services across providers and community resources for long-term care management in a facility setting
  • Provide education on community services and resources
  • Link individuals to community resources and services for community-based integration
  • Engage individuals and families to support sustained participation in services and care plans
  • Assess needs, develop care plans, and coordinate services across providers and community resources to promote continuity of care and long-term outcomes

Requirements:

  • Must meet QP status through one of the specified pathways: full licensure by the NC governing board regulating Human Services professions; fully licensed RN licensed to practice in North Carolina plus four years of full-time MH/DD/SA population experience; master's degree in a human service field plus one year of full-time MH/DD/SA experience or qualifying supervised substance abuse counseling experience; bachelor's degree in a human service field plus two years of full-time MH/DD/SA experience or qualifying supervised substance abuse counseling experience; or bachelor's degree in another field plus four years of full-time MH/DD/SA experience or qualifying supervised substance abuse counseling experience
  • If serving members with LTSS needs, at least two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience
  • Fully vaccinated against Covid-19, influenza, MMR, varicella, and Tdap, with required proof or immunity documentation
  • Must have a valid driver's license
  • Must reside within Trillium's Mid State Region or Southern Region: Anson, Guilford, Montgomery, Randolph, Richmond, Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, or Scotland counties
  • Must be able to travel within catchment as required
  • Must pass a drug test as a condition of employment
