Job Description
RN Case Manager coordinating benefits, care plans, and health services for CVS Health members. Delivering community-based care remotely from New Jersey.
Responsibilities:
- Facilitate delivery of appropriate benefits and healthcare information to determine benefit eligibility
- Promote wellness activities
- Develop, implement, and support health strategies, tactics, policies, and programs
- Support delivery of benefits, overall member wellness, and successful and timely return to work
- Perform assessment, planning, facilitation, care coordination, evaluation, and advocacy
- Coordinate options and services addressing individuals’ and families’ comprehensive health needs
- Communicate with members, providers, and community organizations
- Use available resources to promote quality and cost-effective outcomes
- Collaborate with Community Care teams in a member-centric, team-delivered, community-based care management model
Requirements:
- RN Compact license required
- Minimum 5+ years clinical practical experience with Medicare members; preference for diabetes, CHF, CKD, post-acute care, hospice, palliative care, or cardiac experience
- Minimum 2+ years of discharge planning and/or home health care coordination experience
- Minimum 2+ years Case Management Experience
- Excellent analytical and problem-solving skills
- Effective communications, organizational, and interpersonal skills
- Ability to work independently
- Proficiency with MS Word, Excel, Outlook, PowerPoint, and proprietary applications
- Efficient and effective computer skills, including navigating multiple systems and keyboarding
- Willing and able to obtain multi-state RN licenses if needed; company will provide
- Bilingual preferred
- Certified Case Manager preferred
- Additional national professional certification (CRC, CDMS, CRRN, COHN, or CCM) preferred, but not required
- Associate degree or Nursing Diploma required
- Bachelor’s degree preferred
