Job Description

Clinical Manager leading Cosán’s virtual care coordination teams for at-risk older adults. Managing workflows, coaching staff, and driving clinical performance from a compliant home office.

Responsibilities:

  • Direct and manage daily workflow, queues, and work prioritization to meet productivity, quality, and service-level targets
  • Monitor real-time performance metrics and address volume, coverage, or quality issues
  • Ensure staffing coverage across shifts, including schedule adjustments and PTO coordination
  • Coach, provide feedback, and support care coordination staff
  • Conduct structured one-on-one meetings focused on performance, quality, and development
  • Lead weekly team meetings and cascade priorities and updates
  • Maintain day-to-day engagement, remove barriers, and address issues in real time
  • Foster an accountable team environment supporting engagement, development, and retention
  • Track productivity, quality, and compliance metrics using defined metrics and scorecards
  • Provide documented input for performance reviews, promotions, and compensation discussions
  • Partner with Clinical Operations and HR on performance improvement plans and corrective actions
  • Lead structured team communication through huddles, one-on-ones, and all-hands participation
  • Surface team sentiment, concerns, operational risks, trends, and improvement opportunities to leadership
  • Participate in interviews and provide hiring recommendations
  • Support onboarding and deliver hands-on training
  • Monitor new hire progress and reinforce training
  • Identify development opportunities and support high-potential team members
  • Serve as the first point of contact for employee relations issues
  • Ensure adherence to organizational policies, procedures, and clinical documentation standards
  • Escalate complex employee relations, performance, or compliance issues to the Director

Requirements:

  • Prior experience supervising or managing a production-based team in a healthcare, clinical operations, or call center environment
  • Demonstrated experience managing productivity, quality, and service-level performance
  • Strong working knowledge of care coordination or clinical operations workflows
  • Experience using EMR systems, workforce management tools, and performance dashboards
  • Strong communication, organizational, and interpersonal skills
  • Experience supervising 10 or more direct reports preferred
  • Background in chronic care management or similar care delivery models preferred
  • Experience supporting performance management and corrective action processes preferred
  • Candidates must reside in the United States
  • Private, stationary, HIPAA-compliant workspace within the home
  • High-speed broadband internet required
  • Minimum internet speed of 50 Mbps download / 5 Mbps upload
  • Secure internet connection and ability to protect confidential patient information
  • Working from public spaces, shared co-working environments, or while traveling is not permitted
