Job Description
Clinical Pharmacist delivering medication therapy management and video consultations for Scene Health’s digital healthcare platform. Supporting medication adherence and chronic care through patient and provider engagement.
Responsibilities:
- Conduct patient outreach and manage inbound calls to provide pharmacist services
- Deliver medication therapy management services, including Comprehensive Medication Reviews
- Assess medication appropriateness to optimize therapy and improve clinical outcomes
- Provide comprehensive medication reviews, targeted medication reviews, medication reconciliation, prior authorization review, and other clinical services
- Complete personalized asynchronous video consultations and patient check-ins
- Reinforce clinical information, convert patients into clinical programs, and respond to patient needs
- Assess unnecessary medications, additional medication needs, inappropriate dosing, adverse drug reactions, ineffective medications, and adherence issues
- Document clinical recommendations, therapy responses, and patient care notes
- Send clinical recommendations to provider offices and follow up to ensure recommendations are enacted
- Coordinate with nurses, health coaches, management, product managers, and client-facing staff
- Meet productivity and performance expectations
- Develop or update operational policies and procedures for pharmacy services
- Improve and evolve processes and workflows
- Advise Product and Experience teams on product enhancements
- Hire, train, coach, counsel, and evaluate direct reports as appropriate
Requirements:
- Active Pharmacist License with at least 3 years of pharmacy practice
- Medically fluent in English and Spanish
- Ability to obtain licensure in additional states as needed
- Must be able to serve the elderly population in a clear and fluid manner by telephone
- Excellent communication skills with the ability to develop patient rapport
- Exceptional listening and interpersonal skills
- Ability to work independently as well as in collaboration with a virtual/decentralized team
- Ability to preserve/protect confidential information
- Self-motivated and able to multitask
- Proficient in Microsoft Teams, Outlook, Word, Excel, and other programs
- Must be able to attend meetings and engage with patients on video
- Native fluency in Spanish is preferred
- Experience working in digital health programs and/or working for a health startup is preferred
- Experience working with hospitals or health plans on quality improvement is preferred
- Fluency in Salesforce and additional documentation platforms is preferred
- Sales or call center experience is preferred but not required
- Eligible to work in the United States; visa sponsorship is not available
