Job Description

Family Partner educating parents on navigating child-serving systems of care. Supporting families with mental health and substance use services in North Carolina counties.

Responsibilities:

  • Participate in comprehensive System of Care planning, implementation, coordination, and training related to required core functions within the Region in which it operates.
  • Provide education and support to members and families in learning about and exercising rights and responsibilities.
  • Serve on a multidisciplinary care team to utilize person-centered planning methods/strategies to gather information, to get to know the members and families/legally responsible persons to adequately address specific needs of the individual.
  • Support completion of discovery activities (information gathering and assessment) as a component of the planning process.
  • Assist members/legally responsible persons in choosing service providers, ensuring objectivity in the process.
  • Assist the member/family supported to direct the planning process/plan development, to the extent desired by the member.

Requirements:

  • High School Diploma or GED.
  • Minimum of four (4) years of experience as a primary caregiver for a child or youth receiving Medicaid or State funded BH services.
  • Must have a valid driver’s license
  • Must reside within one of the following North Carolina counties: Anson, Montgomery, Moore, or Richmond.
  • Must be able to travel within catchment as required.
  • Two (2) years paid experience working directly with individuals with BH/MH or IDD conditions. (preferred)
  • Paid experience related to the counties in which they are applying. (preferred)
  • Certified Peer Support Specialist. (preferred)
