Job Description
Family Support Specialist supporting Tennessee caregivers of youth with complex behavioral-health needs. Building engagement, navigating systems, and coordinating peer-informed care at Holon Health.
Responsibilities:
- Partner with caregivers as a peer, using lived experience to build trust and offer hope
- Drive family engagement in the program
- Help families get started, keep appointments, and use the Holon Vibe app
- Reinforce caregiver curriculum and support communication and problem-solving skills
- Help caregivers navigate and advocate across schools, courts, child welfare, community resources, and behavioral-health providers
- Help caregivers identify natural supports and set family goals within the care plan
- Coordinate with the Holon care team, primary care provider, and case manager
- Flag concerns early and keep the family at the center of the plan
- Record participation and progress in the platform
- Uphold guardian consent, youth privacy, and compliance rules regarding voluntary participation, engagement-based rewards, and no inducements
Requirements:
- Lived experience as a biological, adoptive, foster, or authorized relative caregiver of a child or youth with emotional, behavioral, mental health, substance use, or developmental challenges
- 18 or older
- High school diploma or GED
- Tennessee Certified Family Support Specialist (CFSS), held, or eligibility and willingness to obtain within 12 months of hire
- A National Federation of Families Certified Family Peer Specialist (CFPS) credential is accepted as equivalent
- At least 12 months within the past 10 years of paid or volunteer experience supporting families or navigating child-serving systems
- Willingness to work under the supervision of a licensed mental health professional
- Completion of an approved evidence-based family education program, 90 days of role experience, and Family Support Specialist Competency Training within 365 days for the CFSS path
- Comfort with telehealth, smartphone apps, and basic digital documentation
- Tennessee residency
- Reliable transportation and ability to make occasional community or field visits
- Ability to pass background checks required to work with minors and families
- Strong boundaries and cultural humility
