Job Description
Nurse Navigator coordinating oncology referrals, education, clinical trials, and survivorship care. Supporting patients across University of Miami Health System’s multidisciplinary cancer programs.
Responsibilities:
- Serve as the initial point of contact for patients entering the health system
- Triage new patient appointments and assure timely scheduling
- Review outside medical records, test results, and documentation for initial visits
- Assess barriers to care and facilitate referrals to support services and community organizations
- Educate patients and families about diagnoses, treatment plans, treatment options, side effects, survivorship, wellness, and prevention
- Support patients throughout the care continuum, including active treatment, survivorship, chronic cancer management, and end-of-life care
- Coordinate care with multidisciplinary teams, physicians, caregivers, staff, and referring providers
- Facilitate appointments, diagnostic testing, procedures, clinical trial referrals, and continuity of care
- Provide psychosocial screening, shared decision-making support, and referrals to social and financial services
- Attend Tumor Board conferences and coordinate post-conference care
- Track patients through EPIC and ensure required follow-up milestones are met
- Document patient encounters and services provided
- Participate in nurse navigator program development, performance improvement, education, outreach, and staff orientation
- Maintain compliance with University policies and healthcare regulations
Requirements:
- Bachelor’s degree in relevant field
- Valid State of Florida Registered Nurse (RN) license
- Minimum 2 years of relevant work experience
- Ability to maintain effective interpersonal relationships
- Ability to communicate effectively in both oral and written form
- Skill in collecting, organizing and analyzing data
- Proficiency in computer software, including Microsoft Office
- Ability to perform job duties as an independent professional and team player
- Organizational skills sufficient to set priorities and facilitate work team progress
