Job Description
Virtual Care Coordinator supporting patients and clinicians for knownwell’s weight-inclusive metabolic-health services. Coordinating virtual care, patient messaging, monitoring workflows, scheduling, and service recovery.
Responsibilities:
- Partner with Call Center, Patient Access, and Billing teams to coordinate patients’ health coaching and nutrition journeys
- Contact patients to help them navigate care, insurance and payment considerations, service options, and appointment scheduling
- Manage inbound patient messages and escalate clinical questions and concerns to appropriate clinicians
- Own service recovery efforts by developing action plans and coordinating patient communications
- Manage Remote Patient Monitoring program enrollment workflows and device logistics with internal teams and external vendors
- Provide real-time administrative support and technical troubleshooting to clinicians and patients during virtual visits
- Collaborate with the Manager of Virtual Care Operations and cross-functional teams to improve workflows and operational processes
- Deliver a warm, inclusive, and professional patient experience
Requirements:
- High school diploma or equivalent
- 2+ years of experience in a virtual care, patient coordination, or Medical Assistant role
- Familiarity with virtual care workflows in a primary care or chronic care setting
- Prior experience supporting care in a multi-state, remote care delivery model
- Strong communication and organizational skills
- Comfort with remote tools, secure messaging, and EMR platforms (Athena preferred)
- Tech-savvy, adaptable, and comfortable operating independently in a remote setting
- Currently authorized to work in the United States
- No current or future employment visa sponsorship requirement
