Job Description

ChenMed Nurse Practitioner providing remote after-hours telehealth care for seniors. Assessing, treating, and documenting acute and non-acute patient needs via video and telephone.

Responsibilities:

  • Assess acute and non-acute clinical problems through virtual video conference or telephone
  • Perform and document physical evaluations and patient histories
  • Analyze patient data and develop, document, and implement patient management plans
  • Provide health education, physician referrals, case management referrals, and patient/family counseling
  • Plan patient care based on patient population knowledge and protocols
  • Serve as patient advocate and collect comprehensive and focused health data
  • Confer with PCPs and other medical providers to report health data and ensure guideline compliance
  • Use telemedicine to achieve optimal patient outcomes
  • Collaborate with on-call PCPs to support expected clinical outcomes
  • Implement protocols and evaluate progress toward expected outcomes
  • Enhance telehealth practices and systems using effectiveness and success data
  • Communicate through varied formats, tools, and technologies to deliver care across the continuum
  • Provide extraordinary customer service and professionalism
  • Participate in clinical rounds, conferences, risk and quality management programs, and relevant meetings
  • Adhere to departmental goals, performance standards, regulatory requirements, quality-care standards, policies, and procedures
  • Practice under a written or electronic practice agreement
  • Participate in developing telehealth policies, procedures, and protocols
  • Initiate or participate in quality-improvement activities
  • Participate in committees supporting growth
  • Provide feedback on others' practice to improve patient care
  • Coordinate services with other programs
  • Perform other duties assigned by management

Requirements:

  • Graduate of a school of nursing for Advanced Practice Nursing with certification in area of specialty
  • Bachelor’s degree in Nursing (BSN) required
  • Master’s degree in Nursing required
  • Board certification by AANP or ANCC required
  • Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required upon hire
  • Multi-state licensure including Florida and at least two licenses among VA, GA, MI, MO, OH, PA, TN, TX, IL, KY, LA, and KS
  • Minimum 3 years of acute/primary care clinical work experience required
  • Minimum 5 years’ Nurse Practitioner experience highly preferred
  • Urgent care, primary care, or geriatric care experience highly preferred
  • Minimum 2 years’ telehealth work experience preferred
  • Expert business acuity
  • Expert knowledge of job-related functions, practices, processes, procedures, techniques, and methods
  • Knowledge of medical practices to function independently as a certified practitioner and collaborate with licensed physicians and other providers
  • Demonstrated record of achieving clinical performance metrics
  • Technical capability to conduct telemedicine visits under state and federal regulations
  • Excellent clinical judgment and problem-solving ability
  • Ability to prioritize and work under pressure
  • Ability to provide constructive feedback
  • Professional communication and collaboration with physicians, patients, and team members
  • Ability to operate effectively with a multidisciplinary team
  • Proficiency in Microsoft Office Suite, including Excel, Word, PowerPoint, and Outlook
  • Ability and willingness to travel up to 10% for meetings and training
  • Availability for evenings/overnights and/or weekends depending on schedule
  • Ability and willingness to travel to Georgia or other states requiring onsite observation and medical-record review
  • Minimum requirement to work four holidays per calendar year
  • Spoken and written fluency in English
  • Ability and willingness to obtain independent/autonomous NP practice in applicable states
  • Ability and willingness to maintain expertise in the area of practice
