Job Description
Call Center Coordinator conducting remote outreach and screenings for CCNC members. Connecting North Carolina residents with care management, health resources, and referrals.
Responsibilities:
- Conduct continual telephonic outreach to identified and referred members to engage them with care management services
- Receive, retrieve, and manage referrals from callers and data reports
- Complete and document member screening tools and refer clinical needs to appropriate clinical care management team members
- Verify demographic information
- Send educational materials, welcome letters, and other resources to members
- Help identify and address Social Determinant of Health needs
- Assist with scheduling medical appointments and transportation
- Provide information on community resources
- Assist members with care coordination, resource needs, and health care system navigation
- Provide culturally appropriate general education and information
- Monitor incoming calls and assist with member questions or needs
- Meet monthly productivity and role expectations
- Complete assignments within expected timeframes
- Notify the supervisor of referral or assignment issues
- Collaborate with the Care Team to address barriers and improve processes
- Attend departmental and corporate meetings
- Perform other duties as requested
- Work remotely to cover needs across North Carolina
Requirements:
- High School Diploma or GED
- Prior telephonic call center experience desired
- Minimum of two years previous work experience, preferably in a healthcare setting
- Healthcare experience and medical terminology knowledge highly preferred
- Bilingual preferred
- Managed care experience a plus
- Computer skills required including various office software and the internet
- Experience with MS Office software preferred
- Strong initiative and attention to detail

- Knowledge of and experience working in member or clinical data systems
- Must be able to utilize office equipment, computer, keyboard, and phone with or without assistive devices
- Repetitive wrist motion and occasional lifting/carrying of up to 25 pounds
- Hands-on clinical treatment is not permitted when performing duties for the organization
- Ability to comply with department guidelines, company policies, and HIPAA regulations
