Job Description
Care and Outreach Specialist role focused on health system navigation and care coordination for vulnerable populations at CareOregon. Identifying and addressing gaps in care through proactive outreach and collaboration with multidisciplinary teams.
Responsibilities:
- Provide health system navigation and short-term care coordination
- Conduct proactive member outreach to identify and assess care coordination needs
- Determine and address navigation, care coordination and/or resource needs
- Coordinate referrals as applicable
- Collaborate and facilitate services that meet member needs with healthcare providers
- Document all related activities in the centralized care coordination platform
Requirements:
- Minimum 3 years of directly related experience working with vulnerable populations in a healthcare setting
- Experience with Medicaid and Medicare healthcare services
- Understanding of the psychosocial and medical barriers encountered by this population
- Preferred experience working directly or telephonically with similar populations
- Database experience: MMIS, QNXT, Collective, EPIC, and/or other EMR systems
