Job Description

Clinical Outreach Specialist connecting complex-care clients with Vynca’s Enhanced Care Management services. Conducting high-volume outreach, chart reviews, assessments, and care coordination remotely.

Responsibilities:

  • Engage with prospective clients and record all interactions in the Client Relationship Management (CRM) system
  • Make 60–100 outbound calls daily to prospective clients
  • Educate prospective clients about Enhanced Care Management (ECM) services
  • Assess client needs and link clients with the care management team
  • Convey essential information and client preferences to the care team for personalized care planning
  • Assist clients with psychosocial assessments and develop care plans with clear, actionable goals
  • Review medical charts to document client needs and recommend appropriate interventions
  • Apply Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles
  • Participate in weekly Team Operations Meetings to coordinate and optimize care delivery
  • Work remotely and/or in person with Vynca patients and clients
  • Use computers, laptops, electronic health records, and video-conferencing technology throughout the workday

Requirements:

  • Willing and able to work Monday through Friday, 8:30am–5:00pm Pacific Time, with flexibility for potential evenings and weekends
  • Active California Licensed Vocational Nurse (LVN) license, or Medical Assistant certificate issued by a recognized group, or Bachelor’s degree in a related field
  • Minimum of 2 years of telephonic outreach or enrollment experience in a high call volume environment
  • Experience working with complex care clients and ECM Populations of Focus
  • Excellent oral and written communication skills
  • Positive interpersonal skills for effective client engagement
  • Comfortable with technology, including video conferencing, Google Workspace, Slack, and electronic health records
  • Prior experience in a remote role
  • Quiet, secure workspace to ensure member privacy
  • Ability to test internet speed to ensure smooth application functionality
  • Bilingual English/Spanish highly preferred
  • Applicants must reside in Arizona, California, Colorado, Florida, Georgia, Illinois, Nevada, North Carolina, Oregon, Texas, or Washington
  • Background check, potentially including drug test or other health screenings, required prior to employment
  • Patient-, client-, or customer-facing employees must be vaccinated against COVID-19 and influenza; accommodations may be considered

  • Occasional reaching, stooping, bending, kneeling, and lifting items weighing 25 pounds or less
