Job Description

Clinical Transformation Consultant serving as a subject matter expert in customer lifecycle for care management. Supporting successful adoption of Innovaccer technology solutions for clinical outcomes.

Responsibilities:

  • The Clinical Transformation Consultant (CTC) serves as the subject matter expert supporting the customer life cycle from pre-sales to post go-live support with respect to care management activities.
  • The primary responsibility after pre-sales is supporting customers in the successful adoption, optimization, and strategic use of Innovaccer technology solutions to achieve business, regulatory, & clinical outcomes.
  • Deliver high-impact, clinically accurate, and workflow-specific product demonstrations for prospects.
  • Perform demos to align with customer use cases, population needs, and value-based care goals.
  • Support RFPs, RFIs, and solution design sessions with clinical expertise.
  • Participate in discovery calls to understand clinical pain points, workflows, and success metrics.
  • Clearly articulate the clinical, ROI, and operational value of the platform.
  • Deliver ongoing clinical training, workflow support, and best-practice recommendations.
  • Act as a trusted advisor to clinical leaders, care managers, physicians, and operational teams.
  • Monitor adoption, identify gaps, and recommend optimization strategies from a value framework.
  • Support change management initiatives by helping customers adapt workflows and processes.

Requirements:

  • Clinical licensure
  • 3 years or more of experience in clinical practice, care management, population health, or ambulatory clinical operations.
  • Experience using healthcare technology platforms such as EHRs, care management systems, or analytics tools.
  • Strong understanding of value-based care, care coordination, chronic disease management, and quality programs.
  • Excellent communication, presentation, and relationship-building skills.
  • Ability to translate clinical workflows into technical requirements.
  • Comfortable with online & remote schedules
