Job Description
Licensed nurse advocating remotely for Medicare patients at Solace, a U.S. healthcare navigation startup. Coordinating care, navigating resources, and addressing social barriers without providing direct medical care.
Responsibilities:
- Support Medicare members remotely across the full arc of their care
- Use LPN/LVN clinical background while learning Solace’s proprietary platform, tools, and standards
- Build trusting patient relationships through empathy and attentive listening
- Assess patient situations, prioritize needs, and create care plans that maintain progress over time
- Develop care plans addressing clinical and everyday barriers, including food access, transportation to appointments, and in-home support
- Guide patients through complex care coordination, resource navigation, and system or administrative challenges
- Provide non-clinical advocacy rather than direct patient care or medical advice
- Connect with patients by video or phone, never in person
Requirements:
- Current LPN or LVN license in good standing
- Experience working with patients across diverse backgrounds and communities
- Compassion rooted in caring for people and advocating for those who need support
- Ability to organize multiple priorities and remain steady under pressure
- Ability to quickly learn unfamiliar systems, platforms, and tools
- Ownership and ability to independently identify and handle needed tasks
- Applicants must be based in the United States
- Some weekly availability during core business hours, Monday–Friday
- Must be authorized to work in the United States
- Prior care plan creation experience is not required
