Job Description
Remote per-diem Nurse Navigator coordinating oncology appointments, education, referrals, and survivorship care. Supporting patients across University of Miami’s academic health system.
Responsibilities:
- Serve as the initial point of contact for patients entering the health system
- Triage new patient appointments and assure timely scheduling
- Review outside medical records, test results, and documentation for initial visits
- Assess barriers to care and facilitate referrals to support services
- Educate patients and families about diagnosis, treatment options, side effects, wellness, survivorship, and prevention
- Coordinate care with multidisciplinary teams, physicians, caregivers, and referring providers
- Track patients throughout the care continuum and identify bottlenecks or gaps in care
- Support clinical trial eligibility assessment and referrals
- Provide psychosocial screening and referrals to social workers, financial counselors, and support services
- Facilitate shared decision-making and advance care planning
- Attend Tumor Board Conference and coordinate post-conference care
- Maintain communication with the tumor registry
- Document patient encounters and services in EPIC
- Develop patient education materials and community cancer-support relationships
- Contribute to nurse navigator program development and performance improvement
- Orient new staff and attend required meetings and training
Requirements:
- Graduate from an Accredited School of Nursing
- BSN required
- Valid State of Florida Registered Nurse (RN) license
- BLS certification required
- Minimum 2 years of relevant work experience
- Must comply with HIPPA, ACHA, JCAHO, OSHA standards and regulations
- Must complete mandatory in-services and employee health procedures
