Job Description
Registered Nurse Healthcare Advocate guiding U.S. patients through complex healthcare navigation. Creating care plans and addressing social determinants of health remotely via phone and video.
Responsibilities:
- Learn Solace’s proprietary system, tools, technology, and expectations
- Build strong, trusting relationships with patients through listening and empathy
- Identify and prioritize patients’ needs
- Develop comprehensive patient care plans
- Address patients’ social determinants of health, including food resources, transportation access, and support at home
- Guide patients through complex healthcare concerns and help them achieve health and wellness goals
- Navigate resources and resolve patients’ administrative and care-coordination problems via video or phone
Requirements:
- Registered Nurse (RN) license in good standing
- Experience working with diverse patient populations
- Clinical and organizational skills to balance multiple tasks and work under pressure
- Ability to learn new systems quickly and efficiently
- Applicants must be based in the United States
- Must be authorized to work in the United States
- Some weekly availability during core business hours Monday–Friday
- Prior care plan creation experience is not required
- Comfortable supporting patients in a non-clinical capacity without providing direct patient care or medical advice
