Job Description
Registered Nurse preparing Annual Wellness Visit charts and supporting virtual patient care. CareTalk Health operates a nationwide telehealth practice.
Responsibilities:
- Prepare comprehensive Annual Wellness Visit charts to support provider efficiency and CMS compliance
- Conduct face-to-face virtual patient visits via Zoom
- Review and summarize medical, surgical, and family histories
- Review medication lists and document adherence
- Review preventive screenings and immunizations
- Complete cognitive, functional, and depression assessments
- Identify overdue screenings, risk factors, and care gaps before visits
- Communicate concise patient summaries and key findings to providers
- Coordinate with patients to obtain missing or clarified information
- Ensure documentation meets Medicare, CMS, and organizational requirements
- Maintain accurate, timely, and confidential patient records
- Use CT360, RXNT, Zoom, EHR systems, and telehealth platforms
- Measure and support clinical quality, patient engagement, and satisfaction outcomes
- Adhere to HIPAA, CMS, and organizational compliance standards
- Participate in professional development and ongoing training
- Perform independent decision-making, prioritization, and multitasking
- Perform other duties as assigned
Requirements:
- Active, unrestricted RN or LPN/LVN license in a compact state
- 3+ years of clinical nursing experience (bedside, Primary Care, Med/Surg, or Home Health)
- Proven experience providing high-volume patient care in a fast-paced virtual or telehealth clinic environment
- Proficient with EHR systems and telehealth/teleconferencing platforms
- Dedicated, private home workspace suitable for confidential patient interactions
- Strong communication skills and ability to work independently in a remote environment
- Employees must perform all work physically within the United States; international work is not permitted during scheduled work hours
- Windows or Apple (macOS) computer only; no Chromebooks, Linux machines, or smartphones
- Windows 10 or macOS 14 Sonoma minimum
- Intel Core i5 or AMD equivalent processor for Windows, or Apple Silicon for Macs
- 16 GB RAM
- Wired headphones; built-in microphones are not accepted
- Minimum internet speed of 100 Mbps download and 50 Mbps upload
- Ethernet cable connecting the computer directly to the router
- Google Chrome with Amazon Workspaces
- Webcam required for video calls
- Preferred: Experience with Medicare Annual Wellness Visits (AWVs)
- Preferred: Knowledge of CMS documentation and preventive care guidelines
- Preferred: Experience across multiple virtual care programs and platforms
