Job Description
Remote Nurse Practitioner providing serious illness care management for various service lines. Collaborating with medical staff to optimize patient care through virtual assessments and medication management.
Responsibilities:
- Provides fully remote virtual serious illness care management to patients and families throughout
- Consults with medical staff and/or other members of the IDT as indicated to ensure optimal patient care
- Performs virtual assessment of patient/family/caregiver needs
- Identifies, manages, orders, and prescribes medication and equipment as appropriate
- Assesses the caregiver’s ability to meet the patient’s immediate needs and confidence in the patient’s plan of care
Requirements:
- Must have a minimum of 3 years of recent clinical experience
- Substantial experience in hospice, palliative care, and/or acute inpatient care
- Must hold a Masters or Doctors degree in Nursing from an accredited Family or Adult Nurse Practitioner Program
- Must hold national certification, licensure, and prescriptive authority from the appropriate governing body for advanced nursing practice in their home state and in additional state(s) where care is provided
- Must hold and maintain Basic Life Support certificate from an approved organization
