Job Description
Part-time NP/PA conducting in-home health evaluations for CVS Health’s Signify Health. Reviewing histories, performing physical assessments, and providing diagnostic services during member visits.
Responsibilities:
- Provide approximately 45-minute in-home health evaluations
- Engage with members and answer health questions
- Review medications and medical history
- Conduct physical evaluations
- Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry
- Use an iPad and clinical workflow for each evaluation
- Conduct virtual visits as needed
- Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams, and bone density screening
- Build trusted relationships and help improve health outcomes
- Visit health plan members in their homes and connect them to appropriate care
Requirements:
- Active, unrestricted license(s) in coverage area(s)
- Board certification required
- Multi-state licenses preferred
- Nurse Practitioner or Physician Assistant qualification
- Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other airborne allergens or irritants
- Flexibility to travel locally and within licensed states as needed
