Job Description
Per diem LPN providing nursing care to adults with brain injuries and complex health needs. Supporting rehabilitation, medication administration, assessments, and community independence through Vinfen’s nonprofit services.
Responsibilities:
- Participate in comprehensive physical health assessments upon admission and with changes in medical status
- Provide nursing care, teaching, and assistance with daily living activities according to care plans
- Provide wound care, diabetes care, mouth care, foot care, GT/JT care, nutritional management, and catheter care
- Implement IAPs using recovery-oriented, motivational interviewing, harm-reduction, trauma-informed, culturally competent, and strengths-based rehabilitation approaches
- Monitor clients and communicate changes in status to program managers, primary care providers, families/guardians, and outside agencies
- Perform individualized assessments and provide skilled nursing interventions
- Take and document vital signs
- Collect sputum, urine, and stool samples as ordered
- Administer medications and treatments as ordered
- Maintain medical equipment and supplies
- Transport and accompany individuals to medical, dental, and other appointments
- Participate in trainings, supervision meetings, and staff meetings
- Follow company policies, safe work practices, licensing standards, accreditation requirements, and regulations
- Contribute to quality improvement goals
- Use crisis intervention techniques, access on-call backup, and complete incident documentation
- Ensure maintenance and safety of the physical site
- Perform other related duties as required
Requirements:
- Associate's degree in Nursing required
- High school diploma or GED required
- Current L.P.N. license valid in Massachusetts
- Experience working with people with brain injury strongly preferred
- Strong time management and writing skills
- Must be at least 21 years of age
- Current, valid US driver's license held for at least 6 months; later requirements specify at least one year
- Must be able to become CPR certified within 2 weeks of employment
- Must be able to become Safety Care certified within 90 days of employment
- Must successfully pass CORI, reference, and national fingerprinting background checks
- Knowledge of empowerment and self-advocacy techniques
- Sensitivity to cultural, religious, ethnic, disability, and gender issues
- Knowledge of human, legal, civil rights, community, and other resources
- Knowledge of different communication and learning styles
- Knowledge of crisis intervention techniques
- Ability to balance competing priorities
- Ability to make independent judgments and decisions
- Ability to work professionally and confidentially
- Knowledge of personal computer applications and equipment
- Ability to meet physical demands including standing, walking, bending, kneeling, stooping, crouching, crawling, climbing, lifting at least 25 pounds, assisting with daily living tasks, and communicating with clients and staff
- LPN license in the state of service required
