Job Description
Youth Nurse Practitioner providing telehealth assessments, medication management, and coordinated recovery care for Tennessee youth. Supporting clients aged 12-17 with behavioral health and substance use needs.
Responsibilities:
- Conduct health evaluations to assess substance use, co-occurring conditions, and overall functioning
- Diagnose health conditions and create individualized treatment plans
- Prescribe and manage medications, monitor for side effects, and adjust treatment as needed
- Collaborate with the Holon care team and external providers to deliver coordinated, whole-person care
- Serve as a clinical consultant on complex or high-need cases
- Educate clients and families on treatment plans, medications, and recovery support strategies
- Maintain accurate and timely documentation in compliance with ethical, legal, and medical standards
- Participate in team meetings, professional development, and continuous training
- Provide telehealth-based assessments, medication management, and care coordination
- Work closely with hybrid Care Navigator, Case Manager, and Peer Recovery Specialist teams
- Support youth clients aged 12-17 on their recovery journey
Requirements:
- Active and unrestricted NP license in Tennessee
- Active DEA license (and CDS where applicable)
- Several years of experience in behavioral health, addiction medicine, or psychiatry
- Experienced in working with youth populations and their caregivers, specifically those in the 12-17 age range
- Experienced in addiction medicine or psychiatry
- Board certification in specialty preferred
- Strong documentation skills and proficiency in EHR platforms
- Clear and confident virtual communication
- Ability to collaborate in integrated and team-based care models
- Organized, flexible, and capable of navigating complex systems
- Adaptable to evolving schedule needs
- Commitment to person-centered care
- Must support clients aged 12-17 experiencing behavioral health and/or SUD needs
- Tennessee licensure required; additional state licenses are a plus
