Job Description
Certified Diabetes Care and Education Specialist responsible for diabetes education and support. Work with diverse patients remotely to improve health outcomes via phone and webinars.
Responsibilities:
- provide diabetes education support to patients with the goal of improving adherence and health outcomes
- quickly establish rapport and therapeutic alliances via phone with a diverse population of patients
- produce behavior change among patients using motivational interviewing techniques
- present online webinars
- consistently meet call metrics while also meeting quality requirements
Requirements:
- Must have a current certification as a Certified Diabetes Care and Education Specialist from the National Certification Board for Diabetes Educators (NCBDE)
- Must be fluent in Spanish
- Proven experience utilizing motivational interviewing to provide positive behavior change
- Demonstrated ability to receive constructive feedback and then incorporate it into patient calls to drive performance
- Must have a home office for private patient conversation along with high-speed internet
- Able to work independently and manage time efficiently
- Must be comfortable utilizing advanced technology (Electronic Medical Records, Microsoft Office, scripting applications, device data, etc.)
- Demonstrated ability to recognize patient needs and adapt communication techniques to best serve a diverse patient population
- Must have a flexible schedule and be able to commit to a minimum of 20 hours per week to accommodate patients
