Job Description

Care Manager coordinating complex behavioral healthcare for Centene’s Florida Medicaid and Medicare members. Developing care plans, assessing needs, and connecting members with providers and community resources.

Responsibilities:

  • Develop, assess, and facilitate complex care management activities for primarily mental and behavioral health needs members
  • Provide personalized care plans and education for members and their families related to mental health and substance use disorder
  • Evaluate member needs via phone or in-home visits and recommend or facilitate care plans/service plans
  • Perform telephonic, digital, home, and/or other site-visit outreach to assess member needs and collaborate with resources
  • Develop ongoing care plans for high-acuity members and identify providers, specialists, and community resources
  • Coordinate among members, families/caregivers, community resources, and care provider teams to ensure services are accessible
  • Monitor care plans, member status, treatment side effects, complications, and clinical symptoms, and recommend updates
  • Facilitate care coordination with providers and specialists to ensure timely access to care and services
  • Collect, document, and maintain member information and care management activities for regulatory compliance
  • Educate members and families on procedures, provider instructions, treatment options, referrals, and healthcare benefits
  • Provide leadership feedback on opportunities to improve care and quality delivery cost-effectively
  • Perform other assigned duties and comply with policies and standards

Requirements:

  • Applicants must reside in Florida
  • Active Florida mental health/behavioral health licensure or RN with behavioral health experience
  • Master’s degree in Behavioral Health or Social Work, or degree from an accredited school of nursing
  • 2–4 years of related experience
  • Licensed Master’s Behavioral Health Professional, such as LCSW, LMSW, LMFT, LMHC, or LPC, or RN according to state contract requirements
  • Behavioral health experience required
  • Availability Monday–Friday, 8am–5pm Eastern Time
