Job Description
Care Manager coordinating Innovations Waiver services for North Carolina members with intellectual and developmental disabilities. Conducting assessments, person-centered planning, and complex care coordination.
Responsibilities:
- Provide complex care coordination and monitoring to members with primary Intellectual or Developmental Disability and possible secondary mental health, substance abuse, or complex medical/physical health needs
- Conduct assessments, monitoring, and complex coordination of care
- Ensure quality support and adherence to waiver requirements
- Assist with integrated care management through multidisciplinary collaboration and referrals
- Use person-centered planning methods and strategies to gather information and address individual needs
- Educate and support members in learning about and exercising rights and responsibilities
- Complete Discovery activities, including information gathering and assessment
- Inform members and legally responsible people about Medicaid and state-funded services, service options, processes, and natural and community services
- Assist members and legally responsible people in choosing service providers objectively
- Serve children, adolescents, and adults with Intellectual and Developmental Disabilities
- Travel within the catchment as required
Requirements:
- Must meet QP status
- Fully licensed by the NC governing board regulating Human Services professions; OR fully licensed RN licensed to practice in North Carolina by the NC Board of Nursing and four (4) years of full-time experience with the MH/DD/SA population served; OR master's degree in a human service field and one year of full-time MH/DD/SA experience; OR bachelor's degree in a human service field and two years of full-time MH/DD/SA experience; OR bachelor's degree in another field and four years of full-time MH/DD/SA experience
- Substance abuse professionals must have the applicable supervised experience in alcoholism and drug abuse counseling
- If serving members with LTSS needs, at least two (2) years of prior LTSS and/or HCBS coordination, care delivery monitoring and care management experience
- Must have a valid driver's license
- Must reside in North Carolina and within 75 miles of Trillium's Southern Region, including Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, or Scotland counties
- Must be able to travel within catchment as required
- All candidates must pass a drug test as a condition of employment
- Preferred: Bachelor's degree in human services
- Preferred: Experience with the Innovation Waiver or IDD population and writing person-centered plans
