Job Description
Care Manager Registered Nurse at CVS Health coordinating care for members facing chronic medical and behavioral health conditions. Collaborating with interdisciplinary teams and documenting assessments for optimal health outcomes.
Responsibilities:
- Conducting comprehensive assessments to evaluate members’ needs
- Developing and implementing individualized care plans
- Monitoring member progress and advocating for necessary services
- Collaborating with the interdisciplinary care team
- Meticulous documentation of care management activity
- Identifying and connecting members with health plan benefits and community resources
Requirements:
- Candidate must have active and unrestricted Compact Registered Nurse (RN) licensure in the state of residence
- Proficient in Microsoft Office Suite, including Word, Excel, Outlook, OneNote, and Teams
- 3+ years of nursing experience
- 2+ years of case management, discharge planning and/or home healthcare coordination experience
- Confidence working at home / independent thinker
- Bilingual skills, especially English-Spanish
