Job Description
Clinical Pharmacist managing medication therapy and patient interventions for Better Health Group’s medically and socially complex patients. Developing pharmacy protocols, supporting care teams, and ensuring medication safety.
Responsibilities:
- Develop and maintain protocols for prescribing, dispensing, and administering medications
- Conduct research to develop or improve pharmaceutical treatments
- Provide specialized services for patients managing Diabetes, Chronic Obstructive Pulmonary Disease, Congestive Heart Failure, Asthma, or Hypertension
- Participate in quality improvement initiatives
- Participate in the development or revision of pharmacy policies and procedures
- Provide training and support to pharmacy staff and interns
- Maintain current knowledge of new drugs, medical devices, and therapies
- Ensure compliance with federal, state, and local pharmaceutical regulations
- Consult with physicians on drug prescribing practices
- Participate in patient intervention and referral, risk management, and ethical decision-making
- Participate in multidisciplinary team meetings and contribute to patient care plans
- Collaborate with healthcare professionals to plan, monitor, review, and evaluate patient effectiveness
- Educate patients about medications, side effects, interactions, and therapeutic effects
- Review patient profiles to ensure medication safety and efficacy
- Participate in patient care rounds and perform drug therapy consultations
- Manage professional development opportunities within the pharmacy department
- Perform additional duties as assigned
Requirements:
- A doctor of pharmacy degree from an accredited institution
- Licensure as a pharmacist in the state of practice
- 3 years of experience in a pharmacy setting
- Proven experience as a clinical pharmacist
- Excellent understanding of pharmaceutical therapy and direct patient care
- Ability to manage multiple priorities in an expedient and decisive manner
- Proficient with Google Suite (Drive, Docs, Sheets, Slides) and Microsoft Office (Word, Excel, PowerPoint) for real-time collaboration
- Requires standing, walking, pushing, bending, kneeling, and reaching
- Ability to sit for extended periods of time
- Requires corrected vision and hearing to normal range
- Ability to operate a motor vehicle and have own means of transportation
- Demonstrated ability to handle data with confidentiality
- Ability to work cross-functionally with multiple teams and independently with minimal supervision
- Excellent organizational, time-management, and multi-tasking skills with strong attention to detail
- Excellent written and verbal communication skills; comfortable communicating with senior-level leadership, providers, and health plans
- Strong interpersonal and presentation skills
- Strong critical thinking and problem-solving skills
- Results-oriented with a focus on quality execution and delivery
- Appreciation of cultural diversity and sensitivity toward target patient populations
