Job Description

Community Liaison developing healthcare referrals and educating patients for HarmonyCares’ home-based primary care practice. Facilitating enrollment, care transitions, and community partnerships across the assigned territory.

Responsibilities:

  • Conduct inbound and outbound communication and business development
  • Represent HarmonyCares in the local market to educate community partners and potential patients about the company, mission, and service offerings
  • Facilitate patient transitions of care from area facilities and referral partners to HarmonyCares
  • Explain enrollment benefits to eligible patients and caregivers
  • Analyze the market and competitive landscape to differentiate HarmonyCares services
  • Educate the community on HarmonyCares’ personalized, high-quality home healthcare mission
  • Meet or exceed in-person and virtual meeting goals with key decision makers
  • Understand, educate on, and facilitate eligibility, benefit enrollment, and enrollment processes
  • Track performance in CRM against plans and budgeted regional targets
  • Schedule and conduct discussions with patients and caregivers regarding program eligibility
  • Inform patients about accountable care organization or CMS alternative payment model programs and collect voluntary alignment forms when applicable
  • Answer inbound calls from patients, families, and facility staff regarding company business units
  • Triage patient concerns related to care needs, scheduling, and billing to appropriate departments
  • Oversee and document outreach calls and meetings with external sources
  • Comply with marketing, referral-source, beneficiary-notification, patient-outreach, HIPAA, and company policies
  • Maintain communication with sales leaders and local business unit leadership and support staff
  • Perform additional duties as assigned

Requirements:

  • Bachelor’s Degree or 4+ years of B2B sales experience
  • 2+ years of health care related industry sales/marketing experience
  • Must maintain a valid driver’s license and good driving record
  • Excellent organization and problem solving skills
  • Ability to appropriately represent company service capabilities to the targeted referral source
  • Knowledge of sales techniques and territory management
  • Excellent interpersonal communication, presentation and customer service skills
  • Proficiency with Microsoft Office, Word, Excel, Outlook
  • Ability to travel within assigned territory and attend sales meetings as required
  • Excellent written and verbal communication skills
  • Ability to work independently and complete multiple tasks
  • Strong organizational skills
  • Self-motivated and driven individual who does not require significant oversight after initial orientation and training
