Job Description
Social services supervisor leading Inland Empire Health Plan’s community programs and member support. Coaching teams, improving workflows, and advancing health-plan quality outcomes.
Responsibilities:
- Supervise, coordinate, and support implementation of Social and Community Services Programs
- Build a high-performing team
- Manage a team and help break down barriers for Members
- Support development of new workflows and job aids
- Facilitate coaching and provide recommendations for improving performance
- Track and report on operational processes and productivity
- Prepare reports and documents for leadership
- Attend and/or facilitate regular meetings
- Work a flexible schedule including evenings and weekends when needed
- Incorporate IEHP Quality Program goals, including HEDIS, CAHPS, and NCQA Accreditation
- Develop relationships with community-based organizations and leaders
- Drive new initiatives and achieve direct business outcomes
Requirements:
- At least three (3) years of experience in one or more of the following: Supervision/Management, Managed Care/IPA/Clinic Setting, Delivering Presentations/Trainings to varied audiences
- Experience working with State Health Programs (Medi-Cal/Medicare), Case Coordination, Community Health Work/Promotora Programs, Community Outreach, Behavioral Health/Substance Use Disorder Program, Case Management, acute care
- Experience leading a team and operations with accountability for outcomes
- Master’s degree in Social Work from an accredited institution required
- Possession of an active, unrestricted, and unencumbered Clinical Social Worker (LCSW) license issued by the California Board of Behavioral Sciences required
- Knowledge and thorough understanding of Medicare, MediCal, HEDIS, MCAS, DHCS and NCQA
- Knowledge of surrounding social determinants of health
- Knowledge of community resources within the SB/RIV counties
- Knowledge of health care services and CHW role and necessary skillsets
- Understanding and sensitivity to multi-cultural communities and awareness of the impact of unmitigated bias and judgment
- Knowledge of services for Members experiencing homelessness, serious mental illness, substance use disorders, domestic violence, and acute medical health needs
- Knowledge of whole person health and the effect of social deprivation
- Bilingual in one of IEHP threshold languages is highly desirable: English, Spanish, Chinese & Vietnamese
- Excellent verbal, communication and interpersonal skills
- Relationship skills including empathy, motivational interviewing, coaching, social support, and conflict management
- Computer skills including typing, data entry, email, Word, Excel, Outlook, and PowerPoint
- Ability to balance multiple projects, follow ethical and legal rules, identify mandatory reporting situations, and demonstrate planning, organization, time management, and critical thinking
- Strong analytical skills, quantitative goal-setting, data interpretation, leadership, agility, and collaboration skills
- Skilled in evidence-based communication such as Motivational Interviewing or similar empathy-based communication strategies
- Ability to organize and prioritize workload, work under pressure, problem solve, recall information accurately, pay close attention to detail, and work independently with minimal supervision
- Ability to engage team members in settings including by phone, at Members’ homes, in the community, and other settings
- Ability to develop relationships with community-based organizations and leaders, including faith-based communities
- Ability to develop effective relationships with team members while working primarily in the field
- Ability to drive new initiatives and achieve direct business outcomes
