Job Description
Bilingual Intake Specialist supporting home healthcare services enrollment. Educating members and families on home care requirements and coordinating communication between healthcare providers.
Responsibilities:
- Process and manage member referrals received from the Marketing Department
- Educate prospective members and families on home care service requirements
- Coordinate assessments and assist with the enrollment process
- Maintain ongoing communication with members and families throughout enrollment
- Serve as a liaison between members, nursing facilities, physician offices, hospitals, and insurance companies
- Follow up on required documentation and enrollment requirements
- Update daily reports and maintain accurate records in the system
- Ensure all member information is accurately documented and maintained
Requirements:
- At least 1 year of experience as an Intake Specialist, Home Care Intake Coordinator, or Long-Term Care Liaison
- Fluent in English and Spanish
- Strong customer service and communication skills
- Compassionate, professional, and solution-oriented
- Strong follow-up and organizational skills
- Detail-oriented with accurate documentation abilities
- Ability to manage multiple referrals and enrollment cases simultaneously
- Comfortable communicating with members, families, healthcare providers, and insurance companies
