Job Description

Care Manager providing support for frail elders and adults with disabilities at Lakeland Care. Collaborating with an interdisciplinary team for person-centered outcome-based care.

Responsibilities:

  • Serve frail elders, adults with physical disabilities, and adults with intellectual/developmental disabilities
  • Ensure high quality, person-centered, outcome-based care
  • Provide care management and service coordination to LCI members
  • Arrange for provision of services based on comprehensive assessment of member's identified outcomes and needs
  • Monitor provision of services based on member-centered plan per LCI policy and procedures
  • Conduct comprehensive assessment of member's outcomes, needs, and risks
  • Collaborate with RN Care Manager to coordinate acute and primary care services, care transitions, and related follow-up care
  • Develop, coordinate, monitor, and evaluate members’ outcome-based member-centered plans
  • Implement risk mitigation strategies to promote members' health, safety, and independence
  • Educate members of their rights and facilitate their participation in the appeal/grievance process
  • Participate in home visits and care conferences to assess and reassess long-term care needs

Requirements:

  • Certified Social Worker in the State of Wisconsin
  • Minimum of one (1) year experience working with Family Care target populations
  • Four-year bachelor’s degree in Human Services or related field with one (1) year experience working with Family Care populations
  • Four-year bachelor’s degree in any other area than Human Services with a minimum of three (3) years’ experience working with Family Care target populations
  • Working knowledge of computers, computer programs, typing, and data entry
  • Ability to access members’ homes not required to comply with ADA regulations
  • Ability to multi-task and work in a fast-paced environment
  • Ability to lift up to 25lbs
  • Current driver’s license, acceptable driving record, and proof of adequate insurance
