Job Description
Field Nurse Case Manager responsible for coordinating patient support programs at Sentrex Health Solutions. Required to manage patient enrolment and provide medical support in a clinic setting.
Responsibilities:
- Comply with Sentrex policies and procedures.
- Perform tasks associated with enrolling patients and healthcare professionals in accordance with program protocols and applicable service procedures.
- Enter relevant data obtained at various touchpoints during program service delivery.
- Respond to general information requests received by phone or via web platforms about program services, the disease, or the product, using information provided in the program documentation.
- Provide medical support to patients and healthcare professionals, including but not limited to disease-specific training, treatments, and disease management.
- Participate in and prepare for advanced training sessions delivered by the pharmaceutical team on specific treatments.
- Provide telephone support (including inbound and outbound calls as needed) for maintenance treatments and to ensure therapeutic adherence.
- Administer and adhere to case management protocols, including clinical monitoring, data collection, ongoing patient follow-up, compliance, monitoring for treatment interruption or discontinuation, adverse event reporting, and communications with primary healthcare providers.
- Work directly with the primary coordinator and the physician to secure funding to cover the cost of a patient’s treatment and, when necessary, assist in verifying other funding sources.
- When required, attend meetings and conferences with patients to inform them and their healthcare providers about the disease, the product, and case management services.
- Be able to recognize and report adverse events to Health Canada and the Medical Information Service, and enter the data into the source system and third-party systems in accordance with training on current pharmacovigilance requirements.
- Maintain the confidentiality of client and Sentrex information and discuss it only with appropriate staff.
- Liaise with physician offices, pharmacists, infusion service providers, and other parties to deliver coordinated, comprehensive service to patients and physicians and to resolve issues as they arise.
- Participate in continuing education activities held internally and/or externally.
- Enroll patients in the program and perform required follow-ups in accordance with program protocols and applicable service procedures.
- Proactively support and advocate for patients and healthcare professionals during enrollment, insurance renewals, and changes in the patient journey; also facilitate support for patients and physicians to obtain reimbursement.
- Provide ongoing support for questions about the disease or product, relying on the information provided in the program documentation to promote patient therapeutic adherence while considering individual patient needs.
- Follow program protocols to plan the most appropriate next steps to ensure continuous service delivery to patients.
- Visit physician offices at their request and/or at the request of your immediate supervisor.
Requirements:
- Be a licensed registered nurse and a member in good standing of the regulatory body in your province of residence.
- A minimum of two years of clinical experience.
- Experience in immunology, rheumatology, or dermatology is an asset.
- Must be able to provide proof of professional liability insurance and a valid first aid/CPR certificate.
- Strong team spirit and excellent communication skills, both practical and collaborative.
- Experience in a patient support program is a strong asset.
- Proven ability to work effectively as part of a team.
- Bilingual English–French is required in Quebec.
- Must be able to present proof of professional liability insurance and a current CPR certificate.
