Job Description
Part-time physician conducting in-home health evaluations for Signify Health, part of CVS Health. Reviewing medical histories, performing physical evaluations, and identifying care gaps.
Responsibilities:
- Provide part-time in-home health evaluations to health plan members
- Conduct approximately 45-minute visits including medication and medical history review and physical evaluation
- Perform additional diagnostic tests when ordered by the member’s health plan, including diabetic eye exams, spirometry, and bone density screening
- Build trusted relationships and help improve health outcomes
- Identify care gaps and connect members to appropriate care
- Conduct virtual visits as needed
- Provide Signify Health Diagnostic & Preventive Services products
- Travel locally and within licensed states as needed
- Work primarily in members’ private residences
Requirements:
- MD or DO qualification
- Active, unrestricted license(s) in coverage area(s)
- Ability to travel locally and within licensed states, as needed
- Ability to conduct virtual visits as needed
- Ability to safely and comfortably perform duties in private residences that may contain household pets, secondhand smoke, airborne allergens, or irritants
- Multi-state licenses preferred
- Ability to use an iPad and clinical workflow for evaluations
