Job Description
In-home physician conducting personalized health evaluations for Signify Health, part of CVS Health. Reviewing histories, performing exams, and connecting members to appropriate care.
Responsibilities:
- Provide In-Home Health Evaluations in members’ homes
- Engage one-on-one with members and answer health questions
- Review medication and medical history
- Perform physical evaluations
- Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry
- Use an iPad and clinical workflow for each evaluation
- Build trusted relationships and help improve health outcomes
- Visit people nationwide and connect them to appropriate care
- Travel locally and within licensed states as needed
- Conduct virtual visits as needed
- Provide Signify Health services, including Diagnostic & Preventive Services such as spirometry, diabetic eye exams and bone density screening
- Work in members’ private residences and assess health and quality-of-life factors
Requirements:
- Active, unrestricted license(s) in coverage area(s)
- Multi-state licenses (preferred)
- Able to cover the following primary locations: Berks, Bucks, Chester, Delaware, Montgomery or Philly Counties
- Able to travel locally and within licensed states, as needed
- Able to conduct virtual visits as needed
- Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
- Physician qualification as an MD or DO
