Job Description
Full-time Nurse Practitioner or Physician Assistant conducting personalized in-home health evaluations for Signify Health, part of CVS Health. Reviewing medical histories, performing exams, and connecting members to care.
Responsibilities:
- Provide In-Home Health Evaluations in members’ homes
- Engage with people and help bridge gaps in care
- Conduct approximately 45-minute visits
- Review medications and medical history
- Perform physical evaluations
- Perform additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry
- Use an iPad and clinical workflow for each evaluation
- Build trusted relationships and improve health outcomes
- Visit people nationwide to connect them with appropriate care
- Travel locally and within licensed states as needed
- Conduct virtual visits as needed
- Provide Signify Health services, including Diagnostic & Preventive Services products
Requirements:
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multi-state licenses (preferred)
- Ability to be flexible to travel locally and within licensed states, as needed
- Ability to conduct virtual visits as needed
- Ability to safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
