Job Description

Signify Health Nurse Practitioner or Physician Assistant conducting in-home health evaluations for CVS Health members in Pennsylvania. Reviewing medical histories, performing assessments, and providing preventive services.

Responsibilities:

  • Provide In-Home Health Evaluations in members’ homes
  • Build trusted relationships and help improve health outcomes
  • Conduct approximately 45-minute visits
  • Review medications and medical history
  • Perform physical evaluations
  • Conduct additional diagnostic tests when ordered by the health plan, including diabetic eye exams and spirometry
  • Use an iPad and clinical workflow for each evaluation
  • Provide health education and answer members’ questions
  • Visit people nationwide and connect them to appropriate care
  • Travel locally and within licensed states as needed
  • Conduct virtual visits as needed
  • Provide Diagnostic & Preventive Services, including spirometry, diabetic eye exams and bone density screening
  • Meet health plan members in their private residences

Requirements:

  • Active, unrestricted license(s) in coverage area(s)
  • Board certification (required)
  • Multi-state licenses (preferred)
  • Able to cover Berks, Bucks, Chester, Delaware, Montgomery or Philly Counties
  • Able to travel locally and within licensed states, as needed
  • Able to conduct virtual visits as needed
  • Able to safely perform duties in homes containing household pets, secondhand smoke and potential airborne allergens or irritants
  • Nurse Practitioner or Physician Assistant qualification
  • Full-time availability, 40 anticipated weekly hours
