Job Description
Part-time Nurse Practitioner or Physician Assistant conducting in-home health evaluations for Signify Health, a CVS Health company. Reviewing health histories, performing exams, and connecting Rhode Island members to care.
Responsibilities:
- Provide approximately 45-minute In-Home Health Evaluations in members’ homes
- Engage one-on-one with members and answer health questions
- Review medications and medical history
- Perform physical evaluations
- Conduct additional diagnostic tests when ordered, such as diabetic eye exams and spirometry
- Use an iPad and clinical workflow for each evaluation
- Build trusted relationships and help improve health outcomes
- Visit members nationwide and connect them to appropriate care
- Travel locally and within licensed states as needed
- Conduct virtual visits as needed
- Provide Signify Health Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screening
- Work in members’ private residences as the primary workplace
Requirements:
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multi-state licenses (preferred)
- Able to cover Rhode Island
- Able to travel locally and within licensed states as needed
- Able to safely perform duties in homes that may contain household pets, secondhand smoke, and airborne allergens or irritants
- Nurse Practitioner or Physician Assistant qualification
