Job Description
VP of Care Operations & Transformation managing care operations at Chamber Cardio. Responsible for building and leading care teams to enhance patient outcomes through innovative AI solutions.
Responsibilities:
- Build & Lead Next-Generation Care Teams
- Recruit, train, and scale care teams that proactively engage patients through outbound and inbound outreach.
- Drive excellence in communication, documentation, and value-based outcomes.
- Develop workflows for triage, care coordination, education, and escalation.
- Lead the adoption of AI-driven outreach, documentation, quality monitoring, and decision support.
- Ensure care teams are working at top-of-license, shifting repetitive tasks to automation.
- Partner with Product and Data teams to deploy, test, and scale automation that improves efficiency, accuracy, and consistency.
- Own day-to-day care operations: performance dashboards, staffing models, QA programs, and capacity planning.
- Ensure delivery against contractual SLAs and key payer metrics.
- Translate payer requirements into clear operational workflows and accountability structures.
- Stand up care teams with the right structure, capacity, and leadership coverage for live operations.
- Launch operating standards for outreach, documentation, and quality.
- Activate live care programs integrated with Chamber’s platform.
- Implement first-wave AI functionality into frontline workflows to demonstrate efficiency gains.
- Lead an exec-level performance readout on engagement, capacity, and scalability readiness.
Requirements:
