Job Description
Nurse Practitioner or Physician Assistant delivering in-home health evaluations for CVS Health’s Signify Health. Reviewing health histories, performing physical evaluations, and providing diagnostic and preventive services.
Responsibilities:
- Provide approximately 45-minute in-home health evaluations
- Engage with health plan members in their homes and build trusted relationships
- Review medications and medical histories
- Perform physical evaluations
- Conduct additional diagnostic tests when ordered by the health plan
- Use an iPad and clinical workflow for each evaluation
- Visit people nationwide to connect them with appropriate care
- Travel locally and within licensed states as needed
- Conduct virtual visits as needed
- Provide Signify Health diagnostic and preventive services, including spirometry, diabetic eye exams, and bone density screening
- Educate members and answer their health questions
- Do not prescribe medicine, order lab tests, or alter current treatment regimens
Requirements:
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multi-state licenses (preferred)
- Nurse Practitioner or Physician Assistant qualification
- Ability to travel locally and within licensed states as needed
- Ability to conduct virtual visits as needed
- Ability to safely perform duties in homes containing household pets, secondhand smoke, and potential airborne allergens or irritants
- Ability to provide diagnostic and preventive services, including spirometry, diabetic eye exams, and bone density screening
