Job Description

Licensed Supervisor overseeing integrated care-management teams for Trillium Health Resources, a North Carolina behavioral-health managed-care organization. Providing clinical guidance, compliance oversight, and complex-case coordination.

Responsibilities:

  • Oversee a team providing integrated, whole-person care within a managed care environment
  • Supervise staff, including day-to-day responsibilities, performance management, development, and time and attendance
  • Ensure staff use established workflows to complete comprehensive assessments and care plans
  • Establish and maintain a team-based, person-centered approach to care coordination addressing physical, behavioral, and social health needs
  • Monitor team performance metrics, caseload distribution, and quality indicators
  • Ensure quality service delivery and compliance with regulatory and accreditation standards
  • Coordinate care for members with complex needs
  • Provide clinical supervision and guidance using professional licensure to support care planning and clinical decision-making
  • Resolve complex member cases
  • Provide coaching, education, and guidance to staff for continuous improvement and professional development
  • Work and be available at the remote location from 8:30am–5:00pm and travel within the community for meetings as needed

Requirements:

  • Associate's degree (RN only) and three (3) years experience providing care management, case management, or care coordination to the MH/SU/IDD population; OR Bachelor's degree (RN only) and three (3) years experience providing care management, case management, or care coordination to the MH/SU/IDD population; OR Master's degree and fully licensed LCSW, LCMHC, LPA, LMFT, or RN and three (3) years experience providing care management, case management, or care coordination to the MH/SU/IDD population
  • Fully licensed in the State of North Carolina as one of the following: LCSW, LCMHC, LPA, LMFT, LCAS or RN
  • If applying with an Associate’s or Bachelor’s degree, you must be fully licensed in the state of North Carolina as an RN
  • Valid driver's license
  • Must reside within Trillium’s catchment area of North Carolina: Anson, Bertie, Beaufort, Bladen, Brunswick, Camden, Chowan, Carteret, Columbus, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Guilford, Halifax, Hertford, Hoke, Hyde, Jones, Lee, Lenoir, Martin, Montgomery, Moore, Nash, New Hanover, Northampton, Onslow, Pamlico, Pasquotank, Pender, Perquimans, Pitt, Randolph, Richmond, Robeson, Sampson, Scotland, Tyrrell, Warren, Washington, Wayne, Wilson
  • Must be able to travel within catchment as required
  • All candidates must pass a drug test as a condition of employment
  • Preferred: Degree in Human Services field
  • Preferred: At least one (1) year supervisory experience with two (2) or more staff
  • Preferred: Experience with an MCO, Health Plan, or managed-care care management
