Job Description

Supervisor leading remote care coordination teams for Centene, a national healthcare organization. Ensuring compliant member services, continuity of care, team performance, and community-based support.

Responsibilities:

  • Supervise care coordination activities for assigned members to ensure appropriate services and continuity of care
  • Supervise day-to-day activities of care coordination team members and address member/provider issues
  • Monitor and review care coordination documentation, transition-of-care records, and service assessments
  • Assign caseloads based on state and contractual requirements and member needs
  • Collaborate with the care management team to ensure effective, appropriate member care
  • Ensure practices and policies comply with contractual and regulatory requirements
  • Educate and provide guidance and resources to the care coordination team
  • Evaluate team performance and provide feedback on performance, goals, and career milestones
  • Coach the team to improve continuity of care across services
  • Assist with onboarding, hiring, and training care coordination team members
  • Oversee telephonic, digital, home, and other site outreach to assess member needs and collaborate with resources
  • Ensure adherence to service-level, quality, and KPI expectations
  • Lead and champion change within the scope of responsibility
  • Travel to support in-field training, regional team meetings, and leadership meetings

Requirements:

  • Bachelor's degree and 4+ years of related experience
  • Equivalent experience acquired through applicable knowledge, duties, scope, and skill may qualify
  • For Iowa only: Bachelor's degree and 4+ years of experience with populations served, or RN and 6+ years of experience with populations served
  • For Arkansas Total Care plan: driver's license required
  • Child and adult maltreatment check required before hire and on a recurring basis
  • Drug screen required at hire and on a recurring basis
  • Must reside in Arkansas or a border city
  • 20% travel required
  • Remote role within a specific geographic territory with local community travel
  • Experience with Behavioral Health and/or Developmental Disabilities populations strongly preferred
  • Availability Monday through Friday, 8:00 AM to 5:00 PM
