Job Description

Bilingual Scheduler coordinating patient care for healthcare providers remotely. Supporting RNs and ensuring clear communication while maintaining patient privacy standards.

Responsibilities:

  • Initial Report/ Start of Care pulled up daily.
  • Discharge Report to be run daily.
  • Reassessment Report to be run weekly.
  • Assigning RNs to complete the visits.
  • Follow up with RNs on the first day of service reminding them of visits and confirming their completion.
  • Following up on RNs paperwork.
  • Saving all documents in the patient folder.
  • POC to be entered in exchange.
  • MD Order to be sent out.
  • HHA supervision to be sent to HR.
  • Completes Quality Survey JOT form.
  • Mail POC to the patient if needed.
  • Assist RNs in scheduling visits if needed.
  • Assist with authorization for visits as needed.
  • Knowledge and capability to utilize our current software's.
  • Assigning Incident visits to RNs when received via Smartsheet.
  • Duties are subject to change.

Requirements:

  • High school diploma or equivalent required.
  • Fluency in English & Spanish
  • Two years of experience in office work with at least a year of experience in home healthcare preferred.
  • Excellent organizational skills, interpersonal skills, and attention to detail.
  • Proficient with Microsoft Office Suite.
  • Able to coordinate multiple activities concurrently
