Job Description

State Hospital Transition Care Manager coordinating complex care management for individuals with various needs at Trillium Health Resources. Engaging members and facilitating connections to appropriate services while evaluating care plans.

Responsibilities:

  • Engage members in the care coordination/care management process.
  • Provide education about all available services, natural and community supports, treatment options, diagnosis, etc.
  • Provide linkage to needed psychological, behavioral, educational, physical, and dental evaluations.
  • Facilitate referrals for health-related services, community-based resources and social support services, and community reintegration supports.
  • Evaluate the outcome of the care plan and reintegration supports.

Requirements:

  • Must meet Qualified Professional (QP) status.
  • Fully licensed by the NC governing board regulating Human Services professions; OR
  • Fully licensed RN who is licensed to practice in the state of NC by the NC Board of Nursing who also has four (4) years of full-time experience with the MH/DD/SA population served.
  • A graduate of a college or university with a master’s degree in a human service field and has one year of full-time, pre or post-graduate degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has one year of full-time, pre or postgraduate degree accumulated supervised experience in alcoholism and drug abuse counseling; OR
  • A graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time, pre or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has two years of full-time, pre or post bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling; OR
  • A graduate of a college or university with a bachelor's degree in a field other than human services and has four years of full-time, pre or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has four years of full-time, pre or post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling.
  • Additionally, If serving members with Long Term Services and Support (LTSS) needs, the care manager, must meet the minimum QP requirements defined above (A-D) and shall additionally have at a minimum two (2) years of prior LTSS and/or Home and Community Based Service (HCBS) coordination, care delivery monitoring and care management experience. This experience may be concurrent with the years of experience required to become a Qualified Professional.
  • The following immunizations are required for this position: Fully vaccinated against Covid-19, Influenza (annually within Influenza season Nov 1 – March 31), MMR, Varicella and Tdap (Tetanus, diphtheria pertussis). MMR and Varicella - must show proof of vaccination or have blood drawn to show immunity. Tdap - must show proof of one dose. Employees must provide proof of all immunizations prior to their start date or agree to be provide proof of all immunizations within 4 months of start date.
  • Must have a valid driver’s license.
  • Must reside within 75 miles of Wayne County, North Carolina.
  • Must be able to travel within catchment as required.
