Job Description
Care Manager responsible for managing patients in a virtual cardiac rehabilitation program. Collaborating with teams to improve patient health outcomes and reduce cardiovascular risks.
Responsibilities:
- Guide participants in 1:1 video calls and group education sessions through our 3 month online program to help them manage their cardiac related risk factors.
- Development of comprehensive treatment plans that is tailored to the individual risk factors and medical needs of the patients.
- Engage patients weekly to provide coaching, education, emotional support and modify treatment plans through video, voice or text messages.
- Coordinating the approval of treatment plans remotely with Medical Directors.
- Provide progress reports on an as needed basis when symptoms are reported or vital signs are not at goal.
- Share approved treatment plans to referring providers on a monthly basis to provide insight into the patient’s performance in the program.
- Monitor dashboard for adverse events and triage appropriately.
Requirements:
- Minimum Bachelor degree in exercise related science or nursing.
- 1-2 years Past experience in telehealth, remote patient monitoring, chronic care management or virtual care.
- Certifications required: If Exercise Physiologist: ACSM-CEP preferred or willing to sit for the CEP within 6 months of hire
- 1-5 years of prior experience in cardiac education, care management, cardiac rehab and secondary prevention required
- Preferably with a compact RN license or willing to obtain a compact license as well as single state licenses in non participating states.
- Ability to work 15-20hrs/week+.
- Must commit to a set schedule for a minimum of 3 months, with flexibility to adjust schedules on a quarterly basis.
- Tech savvy, with a firm grasp on effective and efficient communication through digital platforms such as Slack, Google Meets, Skype, Zoom.
- Experience executing quality improvement initiatives to lower re-admissions and improve patient outcomes.
- A compassionate and empathetic "people person" who is able and willing to work independently.
- Someone preferably bilingual (Spanish, Mandarin, other languages a plus).
