Job Description

Care Partner coordinating government-funded in-home aged care for Silverchain clients in Coffs Harbour. Managing care plans, budgets, caseloads and home-based client support.

Responsibilities:

  • Manage a caseload of community-based older Australians receiving government-funded aged care services in their homes
  • Assess clients’ needs and develop personalised care plans
  • Manage client funding budgets according to government program guidelines
  • Onboard new clients
  • Proactively manage existing clients using a compassionate, person-centred approach
  • Communicate and collaborate with clinical, scheduling and service delivery teams
  • Visit aged care clients in their homes
  • Advocate to ensure clients’ needs are met
  • Work autonomously from home and within the community

Requirements:

  • Experience in community-based case management is ideal
  • Experience managing government-funded in-home aged care services and budgets is advantageous
  • Current driver’s licence
  • Reliable vehicle with minimum Third-Party insurance
  • Strong verbal and written communication skills
  • Empathy and compassion
  • Problem-solving and critical thinking skills
  • Organisational and time management skills
  • Cultural competence and ability to work with diverse populations
  • Advocacy skills
  • Collaboration and teamwork skills
  • Diploma or Cert IV in Case Management or equivalent (desirable)
  • Successful candidates must complete a National Police check
  • Successful candidates must complete a Work Rights check
  • Successful candidates must confirm COVID-19 vaccination status
