Job Description
Care Partner coordinating government-funded in-home aged care services for Silverchain clients in Coffs Harbour. Managing care plans, budgets and community-based client relationships.
Responsibilities:
- Manage a caseload of community-based older Australians receiving government-funded aged care services in their homes
- Assess clients’ needs and develop personalised care plans
- Manage clients’ funding budgets according to government program guidelines
- Onboard new clients and proactively manage existing clients
- Use a compassionate, person-centred approach
- Collaborate with clinical, scheduling and service delivery teams
- Visit aged care clients in their homes
- Communicate regularly with internal teams to ensure a smooth and comprehensive care experience
Requirements:
- Experience in community-based case management is ideal
- Experience managing government-funded in-home aged care services and budgets is an advantage
- Current driver’s licence
- Reliable vehicle with minimum Third-Party insurance
- Strong verbal and written communication skills
- Demonstrated empathy and compassion
- Excellent problem-solving and critical-thinking skills
- Good organisational and time-management skills
- Cultural competence and ability to work with diverse populations
- Advocacy skills to ensure clients’ needs are met
- Collaboration and teamwork skills for interdisciplinary teams
- Diploma or Cert IV in Case Management or equivalent (desirable)
- Successful candidates must complete a National Police check
- Successful candidates must complete a Work Rights check
- Successful candidates must confirm COVID-19 vaccination status
