Job Description

Care Partner managing government-funded aged care services for Silverchain clients across Sunshine Coast. Assessing needs, managing budgets and coordinating personalised in-home care.

Responsibilities:

  • Manage a caseload of community-based older Australians receiving Government funded aged care services in their homes
  • Assess clients’ needs and develop personalised Care Plans
  • Manage clients’ funding budgets according to government program guidelines
  • Ensure each client’s care requirements are met
  • Onboard new clients and proactively manage existing clients
  • Use a compassionate, person-centred approach
  • Collaborate with clinical, scheduling and service delivery teams
  • Communicate regularly with internal teams to deliver comprehensive care
  • Visit aged care clients in their homes
  • Work autonomously from home and within the community

Requirements:

  • Experience in community-based case management is ideal
  • Experience managing Government funded in-home aged care services and budgets is an advantage
  • Current driver’s licence
  • Reliable vehicle with minimum Third-Party insurance
  • Strong verbal and written communication skills
  • Demonstrated empathy and compassion
  • Excellent problem-solving and critical thinking skills
  • Good organisational and time management skills
  • Cultural competence and ability to work with diverse populations
  • Advocacy skills to ensure clients’ needs are met
  • Collaboration and teamwork skills for interdisciplinary teams
  • Diploma or Cert IV in Case Management or equivalent (desirable)
  • National Police check prior to commencement
  • Work Rights check prior to commencement
  • Confirm COVID-19 vaccination status prior to commencement
