Job Description

Care Partner managing home-based aged care clients, personalised plans and government funding budgets for Silverchain. Working remotely across the Sunshine Coast community with clinical and service delivery teams.

Responsibilities:

  • Manage a caseload of community-based older Australians receiving Government funded aged care services in their homes
  • Assess clients’ needs and develop personalised Care Plans
  • Manage clients’ funding budgets in accordance with government program guidelines
  • Onboard new clients and proactively manage existing clients using a compassionate, person-centred approach
  • Collaborate with clinical, scheduling and service delivery teams
  • Visit aged care clients in their homes
  • Communicate regularly with internal teams to ensure a smooth and comprehensive care experience

Requirements:

  • Experience in community-based case management is ideal
  • Experience with managing Government funded in home aged care services and budgets an advantage
  • Current driver’s licence
  • Reliable vehicle with minimum Third-Party insurance
  • Strong verbal and written communication skills
  • Demonstrated empathy and compassion
  • Excellent problem-solving abilities and critical thinking skills
  • Good organisational and time management skills
  • Cultural competence and ability to work with diverse populations
  • Advocacy skills to ensure clients’ needs are met
  • Collaboration and teamwork skills to work effectively with interdisciplinary teams
  • Diploma or Cert IV in Case Management or equivalent (desirable)
  • National Police check prior to commencement
  • Work Rights check prior to commencement
  • Confirm COVID-19 vaccination status prior to commencement
