Job Description
Expert Coach providing 1:1 support for chronic condition management at Teladoc Health. Responsible for empowering members and facilitating behavior changes to positively impact health outcomes.
Responsibilities:
- Empowers members to effectively self-manage their own chronic conditions
- Assess and identify individual member’s needs to appropriately educate/coach
- Coaching approach congruent with Motivational Interviewing
- Follow scheduling best practices to meet coaching utilization and member needs
- Actively participates in process improvement with a positive attitude
- Provide support to other coaches through mentoring
Requirements:
- Minimum of a bachelor’s degree
- Certification as a Certified Diabetes Care and Education Specialist (CDCES)
- Up to date with the required 75 CEUs in 5 years to maintain CDCES certification
- At least 3 years of professional experience in the healthcare field
- Ability to interpret and incorporate the ADCES 7 DSME/T education model into practice
- Advanced degree in health-related field is desirable
- Bilingual Spanish/English is desirable
